Questionnaires and Referrals
We are dedicated to supporting and collaborating closely with all our Craigslea families to ensure the well-being and educational success of our students. We often receive requests to complete questionnaires, surveys, and provide observation letters to assist student referrals to GPs and paediatricians. These referrals can be made for various reasons.

To streamline this process and ensure efficiency, we kindly ask for up to two weeks notice for the completion of questionnaires and observation letters. This allows us to provide relevant and thorough information to support the referral process.

Additionally, it is essential we obtain consent and permission from parents/legal guardians to share relevant information with the treating professionals. This includes providing details for our department Consent to Share forms including job title, name, company name, email address, and contact details of the treating professional.

Your cooperation in providing timely notice and completing the necessary consent forms will greatly assist us in facilitating these referrals.
